Yes Dr. Gedde, please sign me up for my Health
Advisory Consultation.
I understand that by taking action today:
- I am making an appointment for a personal, one-on-one phone
consultation with Dr. Gedde. During my appointment, we will discuss
my history, symptoms, test results, testing and treatment
options and specific next steps.
Because this is not a doctor-patient relationship (Dr. Gedde is not
taking patients), she is not allowed to
give me medical treatment, order lab tests or prescribe medications.
However, she can coach me on how to request treatment from my
current physician or how to locate a compatible doctor.

My appointment fee is $75 per 30 minutes. The minimum time is 30
minutes, and we can talk for up to 90 minutes.
I’ll submit a $25 deposit today through secure
online payment. Then I’ll pay the rest of my fee directly to Dr. Gedde on
the phone at the end of my consultation.
After my consult, Dr. Gedde will send me an email listing my next
steps.
She'll also send me a link to download an mp3 audio file of our call.
And we'll have a quick phone or email check in about a week later,
to be sure I'm on track with my action steps.
- To get the most out of my consultation, I will send Dr. Gedde background information
on my health concerns before our appointment. For example, I can send a history of my health problems, lab reports, a list of
questions - whatever I feel will be helpful.
I’ll fax this information to Dr. Gedde at 775-766-9681, or email it
to her by replying to the email I receive confirming my appointment.
I can also SUPERCHARGE my consult (at no extra charge) by downloading and filling out Dr. Gedde’s client history form.

To get the MOST benefit possible from our
consultation, I can also collect a three day set of vital signs
using Dr. Gedde's vital signs instructions and vitals signs
chart (this gives key information about my thyroid and adrenal
function):

I will get this information to Dr. Gedde at least a day before
my appointment so she can review it before we talk.

I wish and am invited to pass it along to friends and family as

not for me, I will receive an immediate refund of my deposit and
will owe nothing.

Upon choosing my appointment slot, I’ll go to a page where, on
the right side, I'll enter my name, phone and email, and
a note about my health concerns.
Then I’ll be taken to a page confirming
that my appointment has
been booked.
On the next page, I’ll double-check the time of
my appointment, and once more enter my name and email to be sure Dr.
Gedde can reach me about my appointment.
Then I’ll go to the payment page to place my deposit.
I’ll send Dr. Gedde my additional health information at least a day
before my consultation.
At my appointment time, Dr. Gedde will call
me at the phone number I provide.
- Major credit cards are accepted for my appointment deposit. After I
enter my account details, my payment will be processed safely and
securely by PayPal®. A PayPal account is not required. My account
will show payment to Gedde Whole Health. My information will be used
only for this deposit and will not be kept on file.
- I understand that Dr. Gedde will do her best to help me
reach my health goals, but she makes no promise or guarantee about my
actual results.
Dr. Gedde isn’t taking
patients and won’t be able to prescribe medications or order
tests for me. I’ll make decisions about my medical diagnosis and
treatment together with my physician or other healthcare provider.
